How to get a Health Card at Cebu City Health (2018)

Many employees in Cebu City need to submit a health card to their new employers. It is one of the requirements needed when you get hired.

Here are some useful information and instructions in getting one.

1. Secure all necessary requirements first.
   • Chest X-Ray with film (But they don’t actually get the film, they just need the result)
   • 1 pc. 1×1 ID Picture
   • Completely Filled-out Information Sheet (You can get this at the Help Desk or you may check the picture below)
2. Submit all the requirements to Window 1.
3. Receiving Clerk will verify all your submitted requirements. (If there’s nothing wrong with your documents, then you can proceed to Cashier Window.)
4. Pay ₱10 for your health card – yes just 10pesos! 😉
5. Then, wait for your name to be called before claiming your health card.
6. Affix your signature.
7. And… you’re done! Quick, right?
